For Most this table is optional but is highly recommended for all.

Some Monsters have flaws that require this table, under certain circumstances.
A consistent theme throughout dark fantasy and related genres is the fragility of life and the consequences of actions. While the roleplaying ramifications of a party’s decision can be highlighted through story outcomes, combat ramifications can be a little harder to implement.

Grievous Wounds add grit and realism to combat encounters, bringing the consequences of a difficult fight to the forefront of storytelling.​​​ Grievous Wounds come into play whenever a character is reduced to 0 Hit Points. When a character is reduced to 0 Hit Points, it may trigger Grievous Wounds, but you may use this roll anytime you see fit and it is automated for your conveniance.
These wounds are purely narrative to help add depth to recovery roleplay and sometime consequences if you are unfortunate enough to roll on the Permanent Wound table.​
It is up to you think about how these narratives effects how your character function what hardships it could cause.

If you choose to roll on this table (or if you are required to) you must honor the result there are no test rolls and deciding after Grievous Wounds are typically treatable.
